Assumptions: premiums and deductibles are the official CMS figures for 2026 (Part B standard premium $202.90/mo, deductible $283; Part A deductible $1,736 per benefit period; Part D maximum deductible $615 and a $2,100 out-of-pocket cap). Out-of-pocket figures are illustrative scenarios, not predictions: with Medigap Plan G you pay the $283 Part B deductible plus drug cost-sharing (light ≈ $150, typical ≈ $600, heavy = the $2,100 cap). With no supplement, cost-sharing assumes roughly $3,000 / $10,000 / $40,000 of Part B services at 20% coinsurance (light / typical / heavy), plus one Part A deductible in a heavy year — and it has no cap. Medicare Advantage scenarios assume roughly $300 / $1,200 / $5,000 of copays, under the 2026 in-network out-of-pocket maximum of $9,250. Medigap, Part D and Advantage premiums vary by ZIP code, age and plan; 10-year totals simply repeat 2026 rates and ignore future premium increases, which historically run several percent a year.

Frequently asked questions

What is the Medicare Part B premium in 2026?

The standard Part B premium for 2026 is $202.90 per month, with an annual deductible of $283. Higher-income beneficiaries pay more through IRMAA surcharges, up to $689.90 per month in the top bracket.

What is IRMAA and who pays it?

IRMAA is an income-related surcharge added to Part B and Part D premiums. For 2026 it applies if your 2024 modified adjusted gross income was above $109,000 (single) or $218,000 (married filing jointly). Part B surcharges range from $81.20 to $487.00 per month; Part D surcharges from $14.50 to $91.00 per month.

Is Medicare Part A really free?

Part A has no premium if you or your spouse worked and paid Medicare taxes for at least 40 quarters (10 years). With 30–39 quarters the 2026 premium is $311 per month; with fewer than 30 quarters it is $565 per month. The Part A inpatient deductible is $1,736 per benefit period in 2026.

What is the Part D out-of-pocket cap in 2026?

Once your out-of-pocket spending on covered Part D drugs reaches $2,100 in 2026, you pay nothing more for covered drugs for the rest of the year. The maximum Part D deductible is $615.

Is Medicare Advantage cheaper than Original Medicare with a Medigap plan?

Medicare Advantage usually has lower premiums — the average MA premium is about $11.50 a month in 2026 — but you pay copays and coinsurance as you use care, up to an in-network out-of-pocket maximum of $9,250 in 2026. Original Medicare with Medigap Plan G costs more in premiums (Plan G averages roughly $100–$180 a month) but leaves almost nothing to pay at the doctor beyond the $283 Part B deductible.
